Transportation Request Forms
1. For a PDF of the Out-of-District Transportation Request form, please click here. This form is must be completed prior to the transport of any student out of district (BOCES, BETA, Astor, Devereaux, Kildonan, St. Francis, Benedictine, etc.). This form is to be used by building administrators, guidance counselors, special needs personnel and anyone else in district who is involved with a student's placement.
2. For a PDF of the Request for Transportation to Private and/or Parochial Schools form, please click here. This form is to be used by parents/guardians of Red Hook students requesting transportation to private and/or parochial schools.
