Requirements for Administration of Medications in School
Before any medication can be administered in school (this includes over-the-counter medication), the nurse's office needs:- Written permission from the parent/guardian to administer the medication as specified by the physician
- A written order from the physician clearly stating
- Student's name and date of birth
- Name of medication and diagnosis
- Dosage and route of medication
- Frequency and time of administration
- For PRN (as needed) medications - conditions under which medication should be administered
- Date written
- Prescriber's name, title and signature
- Prescriber's phone number
The medication must be in the original container with the pharmacy label, if applicable. No medication should be brought to school by a student. A parent/guardian must deliver it directly to the health office. This includes over the counter medication as well. This procedure is essential for the protection of students.
